Patient Particulars

• 50 yr old male

• DM, HTN since 11 yrs (on medication)

• C/O Progressive loss of vision since 5-6 yrs

• Aphasia and Memory loss since 1 ½ months

• No h/o weakness of upper or lower limbs

Clinical Examination

• Patient has partial loss of vision

• Difficulty in speech present

• All peripheral pulses palpable

• No carotid bruit

• Power is 5/5 in both upper and lower limbs

Investigations

• All Biochemical parameters within normal limits except raised blood sugar

• ECHO- within normal limits (EF=60%)

• DSE-negative

• CT Brain- Left temporo-parieto-occipital infarct

CT ANGIOGRAPHY

• Right proximal ICA-Near Total Occlusion

• Left ICA- 80% to 90% stenosis

• Left vertebral -80% stenosis at origin

DIAGNOSTIC ANGIOGRAPHY FOLLOWED BY LEFT ICA STENTING
